The purpose of this form is to accurately determine the requirements for replacing your existing lighting with Led Lighting.

Depending on the application, additional levels of information are requested.

Is this a main lobby, workshop, warehouse, parking lot? What is the common area name that can be used as a reference for this portion of the project?

If your facility has 10 distinct bulb types with unique bulb model #'s, then you would select 10 as the total. If you have only 1 bulb type, but it is used in distinct areas with unique times of use, you would create a separate form for analysis for each.
